Self-Certification of Eligibility
In order to participate in this Library of California regional project, each participating library is asked to complete a self-certification form, which will, by its appropriate completion, establish the library’s ability to meet the interim eligibility standards for the Library of California and its eligibility to use the database. Gold Coast Library Network will review forms for completeness and eligibility.
Public K-12 school districts should include subscription contact and technical and population information for the whole district, not for an individual school site. Individual public schools should contact their district to coordinate registration. Do not use this form to register an individual public school.
